Clinic & Outreach Coordinator
Position Summary
The Clinic & Outreach Coordinator will handle day-to-day clinic operations: ordering supplies, inventory management, administrative tasks, perform point-of-testing and counsel of program participants as instructed and clean the office regularly. In addition, outreach responsibilities include taking qualified patients to and from medical/lab appointments, locating missing clients/patients, attending home visits and delivering medication.

Clinic Responsibilities:
Supply Management & Cleaning
Manage and oversee supply inventory requirements:
• Ordering of supplies from approved vendors;
• Monitor weekly inventory requirements and coordinate with head office
and suppliers to order appropriate volumes of supplies, stock the clinic and ensure receipt of deliveries;
• Manage administration and reconciliation of gift cards and cell phones;
Cleaning the office space between patient screenings and at the end of each workday
Screening Services
• Ensure all program participants understand and consent to the full intake process;
• Register patients in database and ensure data requirements are entered appropriately;
• Contact the Ministry to validate health card information for program participants;
• Point-of-care testing, intake coordination and scheduling of medical/lab appointments;
• Ensure consistent education (disease and risk factors) are provided to all participants;
• Provide gift card & cash incentives to qualified patients with appropriate documentation;
• Promote program incentives (e.g., treatment – cell phone/gift cards, friend referrals, etc.)
• Locate missing clients in the community to complete full intake process
Administrative Support
• Provide office support with other administrative functions (e.g., uploading faxes, patient
adherence calls, telephone registrations, reviewing and updating program data, etc.);
• Locate missing clients in the community to complete full intake process
Outreach Responsibilities:
Outreach Services
• Bring qualified patients to and from lab/medical appointments;
• Locate missing clients in the community to complete full intake process;
• Deliver medications to patients as needed (e.g., weekly);
Oversee medication remittance and liaise with clinical staff to provide counselling;
• Attend outreach screenings and support outreach peers as required
Other:
• Perform clerical work such as emailing, photocopying, faxing and data entry;
• Work to resolve any issues related to clinic and patient services;
• Other duties as required
Qualifications:
• Combination of post-secondary education and/or equivalent work experience working within marginalized/(PWUD) communities;
• Ability to work between the hours of 8:00 a.m. and 5:00 p.m. Monday to Friday;
Competencies:
• Fluent in English; proficiency in other language is an asset;
• Strong written and oral communication skills;
